Overview

We are looking for a PRN RN to work in our Homecare department. Work hours will be on a as needed basis. You will be helping cover for vacations, sick time, high census and any other time when needed. The Homecare department is open days, Monday – Friday, and they do take call on the weekends. As a PRN staff you will be required to work (1) 8 hour shift per month, (2) weekends per year and (1) holiday per year. As a PRN employee you will receive a 10% PRN premium. 

Qualifications

Must hold an active license to practice nursing in the State of Iowa. The position requires at least an Associate’s degree in nursing. RN employees are required to have or obtain a BSN within 5 years after their start date with UnityPoint Health – Finley Hospital. Within 1 year of hire, need to obtain RN licensure with the States of Wisconsin and Illinois. CPR certification is required. Must have valide driver’s license and proof of personal auto insurance coverage. 

Responsibilities

Provides professional nursing care in the home by utilizing all elements of the nursing process. Collects health data, demonstrates knowledge and application of patient assessment skills, performs initial and ongoing assessment of patient and family/caregiver. Analyzes the assessment data to determine nursing diagnosis and develops plan of care. Implements the interventions identified in the plan of care in a safe, timely, and appropriate manner. Evaluates the patient’s progress toward attainment of outcomes on an ongoing basis. Completes all required documentation accurately, thoroughly, and on time. Educates and involves patients, families, and caregivers to promote self-care activities. Coordinates care with other disciplines and provides supervision of nursing assistants. Identifies and coordinates community resources and reimbursement resources necessary to support the plan of care. Promotes continuity of care through discharge planning, case management and advocacy.

Supervisor RN-Intensive Care Unit

Overview

We are looking for a full-time Supervisor RN to work in our ICU department. Work hours will be primarily days, Monday – Friday, for 80 hours per pay period, and as needed. You will be required to flex to other shifts as needed. 

 

Assists in managing the day to day operations of the unit utilizing quality, cost effective patient care principles.  Supervises, supports and develops unit staff.  Consistently utilizes a holistic approach (considering physical, psych/social, spiritual, educational, safety, and related criteria, appropriate to the age of the patients served in the assigned service area). Acts in accordance with DNV Primary Stroke Center guidelines and educational requirements therein.

Responsibilities

Patient Care/Care Coordination

  • Makes visits to the homes of patients requiring home health occupational therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.
  • Evaluates and assesses the patient’s level of functioning by applying appropriate tests.
  • Develops specific occupational therapy procedures as well as functional activities in accordance with written physician plan of treatment and patient evaluation.
  • Develops and administers plan of treatment including environmental modifications.
  • Oversees use of treatment modalities, prosthetic devices and therapeutic appliances. Uses equipment and supplies safely, effectively, and efficiently.
  • Ensures quality and safe delivery of services to patients according to the care plan.
  • Performs and directs therapy treatment as ordered.
  • Provides the services that are ordered by the physician as indicated in the plan of care and notifies physician of any changes in patient’s condition.

  • Communicates with all physicians involved in the plan of care and other health care practitioners related to the current plan of care.
  • Identifies patient’s risks for hospitalization and/or emergency department use and proactively intervenes to prevent adverse events.
  • Supervises home health aide and certified occupational therapy assistant staff and documents supervision in the patient medical record.

Home Care Performance Criteria

  • Delivers quality care and completes clear, concise and accurate clinical documentation of care provision in accordance with home health conditions of participation (CoPs), various payer home health coverage criteria, accreditation standards, professional standards for discipline/credential and the home health plan of care.
  • Participates in clinical review of records to monitor quality and accuracy, appropriate utilization, appropriate involvement of interdisciplinary team members, appropriate referrals and coordination of care.
  • Participates in peer review and other quality assurance activities, as assigned.
  • Uses appropriate infection control procedures and safety measure to protect employees and patients.
  • Participates in case conferences, in-service programs and other meetings required to ensure coordinated and comprehensive care.
  • Reviews all patient assignments prior to scheduling first visit and accepts patient assignments only for which qualified and competent in any specific care/technical skill/procedure needed.

MRI Technologist

TITLE: MRI Technologist

DEPARTMENT: Radiology

 

POSITION STATUS/HOURS:      

Part-time .80 FTE (64 hours per pay period), shifts varying between start time of 6:30 a.m. and ending time of 11:00 p.m. plus call for X-ray and CT. Rotational on-call shifts will include weeknight, weekend, and holiday on-call hours. Call rotation will be every 6th weekend, with 1 day at most of call a weeknight. Generally, a day off after a call shift. Response time is 30 minutes. Benefit eligible position.

JOB SUMMARY:

On March 19, 2018 we opened our brand new, state of the art, Radiology Suite as part of our new building and renovation project. Services offered at Grant Regional include: general radiology, bone density, ultrasound, mammography, nuclear medicine, CT, MRI, and fluoroscopy. Come join our team!

Additionally, MRI tech will be expected to perform X-Ray, C-Arm, and fluoro cases as needed. Will be cross trained, if not already, to perform bone density and CT. Call will be for X-Ray and CT only. Currently we do not have MRI call, but could change depending on future growth. Call rotation will not start until cross-trained in CT.

 

GENERAL RESPONSIBILITIES:

Duties would include, but not be limited to the following:

  • Operate MRI equipment to produce quality MRI’s of the body for diagnostic purposes, as directed by Radiologist.
  • Position patient on examining table and adjust immobilization devices to obtain optimum views of specified areas of body requested by physician.
  • Explain procedures to patient to reduce anxieties and obtain patient cooperation.
  • Maintain patient records, monitor equipment functions monitor patients while thy are being scanned, and ensure the image captured is clear and readable.
  • Follow specific procedures to ensure your safety and the safety of other workers and patients.
  • Function as the first line interface with customers in the successful accomplishment of their imaging needs.
  • Participate in Continuous Quality Improvement activities.
  • Operate radiographic equipment to produce quality radiographs of body for diagnostic purpose, as directed by Radiologist.
  • Practices radiation protection techniques, using beam restrictive devices, patient shielding skills, and knowledge of applicable exposure factors to minimize radiation to patient and staff.
  • May operate mobile radiographic equipment in operating room, emergency room, or at patient’s bedside.
  • Maintain a positive and professional atmosphere for patients.
  • Transfer and properly position patients for examinations and provide immobilization as required and protection in accordance with prescribed safety standards.
  • Select and operate equipment, selecting proper technical factors on an individual patient bases.
  • Use film markers correctly and consistently.
  • Assist physicians in performance of examinations.  Assist in performing first aid as necessary.
  • Practice sterile techniques and prevents cross-contamination.  Maintain orderliness and cleanliness.
  • Secure and return supplies and maintains room stock.
  • Report equipment failure to supervisor using proper forms and maintaining appropriate records.
  • Responsible for delegated areas of authority in the performance of routine or special procedures.  In the absence of the supervisor or senior technologist, may be designated as acting supervisor.
  • Perform and obtain complete patient history, proper documentation, and consents.
  • Purge and digitize film following required guidelines.
  • Complete image release processes appropriately.
  • Assist in scheduling exams and procedures.

 

QUALIFICATIONS:

  • Graduate of an AMA approved school of Radiologic Technology, either college affiliated or hospital trained.
  • Registered with the American Registry of Radiologic Technologists (ARRT) as an MRI Technologist.
  • Current State of Wisconsin license as a Licensed Radiographer.
  • Current CPR certification.
  • Previous MRI Technologist Experience Required.
  • Knowledge of anatomy; including cross-sectional anatomy, physiology, and medical terminology.
